Do I Need Physiotherapy After ACL Reconstruction?
Updated: Oct 4
ACL rehabilitation is a long, progressive process and professional input can help individualise exercise and progression. This article explains what may be contributing, what to monitor and how rehabilitation can address it without relying on a one-size-fits-all timeline.
A useful assessment considers range, swelling, strength, movement quality, goals and surgical restrictions. The exact priorities depend on graft choice, whether there were additional procedures such as meniscal repair, the surgeon’s restrictions, the stage of rehabilitation and the activities you want to return to.
It is also important to look at the knee’s response over the following 24 hours. A session that looks fine at the time may still have been too much if swelling, pain, stiffness or movement quality are clearly worse later that day or the next morning.
Rehabilitation usually combines progressive strength work with restoration of knee movement, gait and single-leg control. As capacity improves, exercises can become heavier, faster and more specific to running, jumping, landing, deceleration and sport. Progression should be earned through good response and objective improvement rather than rushed to meet an arbitrary week.
Modern ACL rehabilitation is progressive and criteria-based. Exercise is the mainstay, while pain, swelling, range of motion, strength and movement quality help determine when load should increase.
When should you get the knee reviewed?
Seek clinical review if you develop a new episode of giving way, a locked knee, rapidly increasing swelling, worsening loss of movement, significant calf pain or swelling, wound concerns, fever, or a substantial deterioration that does not settle. Sudden chest pain or shortness of breath requires urgent medical assessment.
