Falls Prevention Exercises for Older Adults

Falls are a major concern for many older adults. A fall can lead to injury, but even when no serious injury occurs, it can significantly affect confidence. After a fall, people often become more cautious. They may: • walk less • avoid stairs • stop going outside alone • hold onto furniture • reduce exercise Unfortunately, reduced activity can contribute to further loss of strength and balance, which may make future falls more likely. Exercise can help address some of the physical factors associated with falls risk, particularly leg weakness, reduced balance and poor confidence. The key is using exercises that are safe, progressive and appropriate for the individual.
Why do older adults fall?
Falls rarely have one single cause. Possible contributing factors include: • reduced leg strength • poor balance • reduced vision • medication • dizziness • blood-pressure changes • neurological conditions • environmental hazards • poor footwear • reduced confidence Because there are many possible causes, repeated falls should not simply be treated as a normal part of ageing. A medical and/or physiotherapy assessment may be appropriate. Can exercise prevent falls? No exercise programme can guarantee that someone will never fall. However, exercise can improve some important physical abilities related to falls risk, including: • strength • balance • stepping • walking • confidence Falls-prevention programmes often combine strength and balance training rather than focusing on only one area. Why is strength important for falls prevention? Strong legs help with: • getting out of a chair • walking • stairs • stepping onto kerbs • recovering from a stumble • controlling your body when balance changes If the legs are weak, everyday movements require more effort. Strength training can help improve that capacity. Exercise 1: Sit-to-stand Sit-to-stand is one of the most useful functional strength exercises. It trains muscles including the: • quadriceps • gluteals • trunk To perform it: 1. Sit towards the front of a sturdy chair. 2. Place your feet comfortably underneath you. 3. Lean slightly forwards. 4. Push through your legs to stand. 5. Lower back down with control. Use your hands if needed. As you become stronger, gradually reduce how much you rely on them. How can sit-to-stand be progressed? You can make it harder by: • using less hand support • increasing repetitions • using a slightly lower chair • adding a light weight The progression should match your ability. Exercise 2: Calf raises Calf raises strengthen the muscles at the back of the lower leg. These muscles help with: • walking • pushing off the ground • balance • stairs To perform them: 1. Stand near a stable support. 2. Keep your feet about hip-width apart. 3. Rise slowly onto your toes. 4. Lower back down with control. Start with both hands supported if needed. How can calf raises be progressed? Progress by: • reducing hand support • increasing repetitions • adding resistance • progressing towards single-leg work where appropriate Exercise 3: Supported squats Squats strengthen the legs and hips. You do not need to perform a deep squat. Start with a comfortable range. 1. Stand with a stable support nearby. 2. Push your hips slightly backwards. 3. Bend your knees into a small squat. 4. Keep the movement controlled. 5. Return to standing. Use the support as much as needed. How can squats be progressed? You can gradually: • increase the depth • reduce hand support • add resistance • increase repetitions Avoid forcing a range that causes sharp pain. Exercise 4: Step-ups Step-ups help train the muscles needed for stairs and kerbs. Use the bottom stair or a low step. 1. Hold a stable support if needed. 2. Step up using one leg. 3. Bring the other leg onto the step. 4. Step back down with control. Start with a low step. Progress height gradually. Why are step-ups useful for falls prevention? Because real life regularly involves stepping onto: • kerbs • stairs • door thresholds • uneven surfaces Step-ups train both strength and control. Exercise 5: Feet-together balance This is a simple starting balance exercise. 1. Stand near a kitchen worktop or stable support. 2. Bring your feet closer together. 3. Stand upright. 4. Use hand support if needed. As confidence improves, reduce support gradually. Do not close your eyes unless a professional has specifically advised that this is appropriate for you. Exercise 6: Tandem standing Tandem standing means placing one foot directly in front of the other. This narrows your base of support and makes balance more challenging. Start near a stable surface. Progress from: • both hands supported • one hand • fingertip support • no hands where safe You can also practise with either foot in front. Exercise 7: Weight shifting Weight shifting helps practise moving your centre of mass safely. 1. Stand with support nearby. 2. Slowly shift your weight from one leg to the other. 3. Keep your feet on the floor. 4. Move slowly and deliberately. This can later progress towards stepping. Exercise 8: Forward stepping Practising stepping is useful because recovering from a loss of balance often requires taking a quick step. 1. Stand near a stable support. 2. Step one foot forwards. 3. Transfer your weight. 4. Return to the starting position. 5. Repeat on the other side. Progress by reducing hand support where appropriate. Exercise 9: Sideways stepping Sideways stepping challenges balance in a different direction. Walk several controlled steps sideways while keeping a stable support nearby if needed. This can help train: • hip strength • coordination • lateral balance Exercise 10: Turning practice Turning is a common situation where people feel unsteady. Practise: • slow turns • turning both directions • changing direction while walking Take several smaller steps rather than pivoting quickly on one foot. Why should balance exercises include movement? Because real-life balance is dynamic. You need balance while: • walking • turning • stepping • reaching • getting out of a chair Standing still is only one part of balance. This is why a complete programme should eventually include movement-based challenges. Can walking alone reduce falls risk? Walking is valuable but may not be enough on its own. Walking improves: • endurance • mobility • general activity But it may not sufficiently improve: • leg strength • reactive balance • stepping ability A combination of walking, strength and balance work is often more useful.
How often should falls-prevention exercises be done?
Consistency is important. Some balance exercises may be practised regularly. More demanding strength exercises may need recovery between sessions. The exact frequency depends on: • ability • health • exercise difficulty • fatigue • falls risk A physiotherapist can create an appropriate schedule. Should I exercise if I use a walking stick? Yes, if the exercises are appropriate for you. Using a walking stick does not prevent you from strengthening your legs or practising balance. You may simply need: • more support • simpler exercises • supervision The aim is to work from your current ability. Can I do these exercises if I have arthritis? Many people with arthritis can still perform strength and balance exercises. They may need modification. For example: • use a higher chair • reduce squat depth • use a lower step • reduce repetitions Exercise should be adjusted rather than automatically avoided. What if I have knee pain? Some discomfort can occur with strength training. However, sharp pain or a large flare that does not settle may mean the exercise needs modification. Try: • smaller range • fewer repetitions • more hand support • lower resistance Then progress gradually. What if I am afraid of falling during the exercises? Use support. You can practise near: • a kitchen worktop • sturdy chair • wall You may also benefit from supervision initially. The purpose of balance training is not to put you in situations where you are likely to fall. It is to challenge balance safely. Should I do balance exercises barefoot? This depends on the exercise and your individual situation. If you usually wear supportive footwear when walking, it may be sensible to exercise in similar footwear. People with reduced sensation or foot problems should take extra care. Ask your healthcare professional if unsure. Can strength improve after 70 or 80? Yes. Older adults can still gain strength with appropriate resistance training. Age does not make exercise pointless. The key is: • appropriate starting level • consistency • progression • safety What if I have not exercised for years? Start simple. You might begin with: • sit-to-stands • supported calf raises • feet-together balance • short walks You do not need to start with complicated exercises. As you improve, the programme can become more challenging. What if I have already had several falls? Seek assessment. Repeated falls can have multiple causes. Physiotherapy can assess: • strength • balance • walking • mobility But other factors may also need review, including: • medication • vision • dizziness • medical conditions Falls should not automatically be treated as an exercise-only problem. What if I feel dizzy? Dizziness can increase falls risk and may need medical assessment. Do not simply continue challenging balance if you have unexplained dizziness. Speak to your GP or healthcare team. Seek urgent help if dizziness is associated with: • facial weakness • speech problems • sudden weakness • severe loss of coordination Why should exercises be progressed? Your body adapts. If you repeat the same easy exercises indefinitely, they may stop providing enough challenge. Balance exercises can progress by: • narrowing stance • reducing hand support • increasing reach • adding stepping Strength exercises can progress by: • adding resistance • increasing repetitions • using harder variations Progress should happen gradually. What about group exercise? A structured group can provide: • routine • supervision • progression • social interaction • confidence A physiotherapist-led strength and balance class can also adapt exercises according to ability. When should I seek professional assessment? Consider assessment if you: • have fallen repeatedly • feel increasingly unsteady • have unexplained dizziness • have rapidly declining mobility • use furniture to move around the home • have significant weakness • are afraid to exercise independently The right programme depends on understanding what is contributing to the problem. The main message Falls prevention is not about one magic exercise. It is about improving the physical abilities that help you move safely. A good programme usually includes: • leg strength • balance • stepping • turning • walking • confidence The exercises should be safe and should progress as your ability improves.
